Best hospitalisation insurance in Belgium (2026)

Updated · figures checked 2 November 2026

The short version

  • Belgian statutory insurance pays only part of a hospital bill, and a single room brings supplements. A hospital plan covers what is left, and some employers already provide one.
  • Check first whether your employer already gives you a group plan. If not, the cheapest route is usually the plan sold by your own mutuality, which is only open to its members.
  • Want English help: Partenamut or Helan, which sell the same Hospitalia range insured by MLOZ Insurance. Partenamut has an English-speaking expat desk; Helan has full English product pages.
  • Want a plan independent of your mutuality, with a published price list: Ethias. Want a single room in the expensive Brussels hospitals: look at the AG Insurance SL option through a broker.
  • Published prices are not like-for-like: some are single-room plans, one is a double-room base plan, and the private tariffs change with your region. Compare the same room type for your own age and postcode.

A night in a Belgian hospital is not free, even with a mutuality. Statutory insurance pays part of the official tariff, but you still pay your own share, and if you ask for a single room, doctors may charge fee supplements on top of the legal rate. Hospitalisation insurance (assurance hospitalisation in French, hospitalisatieverzekering in Dutch) pays those amounts, and usually also part of the costs in the weeks before and after a stay.

This guide is for newcomers who have just joined a mutuality, or are about to, and wonder whether they need a hospital plan and which one. Some employees already have one through their employer, so the first question is not which plan to buy, but whether you need to buy one at all.

If you do, three things decide the choice: whether you want a single room or are happy with a shared one, which mutuality you belong to (because mutuality plans are reserved to members), and whether you need to read the cover in English. Price comes after, and the published prices are harder to compare than they look.

How hospital insurance works in Belgium

You need a mutuality first

Statutory health insurance runs through a mutuality; joining one is required to get healthcare reimbursements. (source, checked 2 November 2026) A hospital plan comes on top of that, it does not replace it. Mutuality plans are only sold to members of the same fund. AG Insurance requires you to live in Belgium and be affiliated to a Belgian mutuality before it will insure you. DKV cuts its refund by half when the statutory insurance pays nothing on the bill. In practice, join a mutuality first; our guide to the best mutuality for expats explains how.

Two kinds of seller

There are two families of hospital plans, supervised by different authorities.

The first is the mutuality plan. Each union of mutualities sells hospital insurance through its own mutual insurance company (société mutualiste d’assurances). Mutuality hospital plans are sold by sociétés mutualistes d'assurances listed by the OCM/CDZ; MLOZ Insurance (Hospitalia), for example, is listed under code 750/01. (source, checked 2 November 2026) The OCM lists six SMAs offering insurance: CM (150/01, 150/02), neutral (250/01 HospiPlus, 250/02 Neutra), socialist (350/01-03 Solidaris), liberal (450/01 Hôpital-Plus) and free mutualities (750/01 MLOZ Insurance). (source, checked 2 November 2026) The important practical point is that the fund you belong to is the seller, while a separate company carries the risk. Partenamut, Helan and Freie Krankenkasse all sell Hospitalia, and Hospitalia belongs to MLOZ Insurance, not to any of them individually.

The second is the private insurer. Private hospital insurers such as DKV Belgium are authorised Belgian insurance undertakings listed by the FSMA for class 2 (sickness). (source, checked 2 November 2026) You can buy these plans whatever your mutuality, through the insurer or a broker. You can check any insurer or broker on the FSMA register.

Your employer may already cover you

Hospital insurance can also come through your job. AXA Belgium, for example, states on its health insurance page that it does not sell hospitalisation insurance to private individuals: employees get it through their employer, and self-employed people through their company. Ask HR before you buy anything: whether you are covered, whether your partner and children can be added, and what happens to the cover when you leave. If you already have a group plan, check what a second plan would add before paying for it.

Waiting periods, entry age and pre-existing conditions

Three rules decide whether you get paid in your first year.

Waiting period. Most plans do not pay for anything except accidents during the first months. AG Insurance has none once you are medically accepted. Helan states that the waiting period is waived after an accident, or if you had similar cover before joining, so keep proof of any previous policy.

Entry age. Several plans close to new members above a certain age, and mutuality plans set your premium bracket by your age when you join. Joining younger keeps you in a lower bracket for life.

Pre-existing conditions. DKV lists conditions or symptoms present at signature among its main exclusions. Partenamut keeps the same premium but does not refund single-room supplements for a condition you already had. Answer the medical questionnaire carefully and keep a copy.

Prices: why the figures in the table are not like-for-like

Each price in the comparison table is one profile and one formula, and they differ in ways that matter:

  • Room type. Solidaris’s figure is its base plan, which covers a double room; single-room cover needs a paid option. The other published figures are for plans that cover a single room.
  • Region. Ethias prices by region, and its Wallonia tariff is the highest, Flanders the lowest. We used the Brussels column.
  • Age rule. Mutuality plans price by age when you join; Ethias prices by your current age category.
  • Annual ceilings. The Liberal Mutuality’s plans are capped per year; Hospitalia Plus is not.
  • No published price. DKV and AG Insurance only quote through a simulator or a broker.

So a lower figure is not automatically a cheaper plan for the same cover. Compare the same room type, for your age and postcode. Premiums also rise over time: Ethias, for example, states that its premiums are indexed every year.

The providers compared

1. Partenamut (Hospitalia) 6.9/10

Ranked 1 of 7 rated providers in this guide.

Best for: Expats in Brussels or Wallonia who want English help and a mid-priced single-room plan.

Partenamut sells the Hospitalia range to its own members. The insurer is MLOZ Insurance, the mutual insurance company of the free mutualities; Partenamut acts as its agent. The three formulas are Hospitalia Plus / Care / Smart.

The price we recorded is €26.90/month (€322.80/yr) - Hospitalia Plus, age 25-49, joined before 46 (2026, 10% tax incl.). That is a single-room plan, so compare it with Ethias or the Liberal Mutuality, not with the Solidaris double-room base plan. An optional serious-illness guarantee adds a small monthly amount on top.

Single room: Yes, fees up to 300%; €150 yearly deductible in some hospitals. Before and after a stay, costs are covered for 60/120 (Smart), 60/180 (Care, Plus). Waiting period: 3 months. Maximum age to join: 67 (Hospitalia Plus).

For an English speaker, Partenamut’s main strength is its Business & Expats service: English expat pages that explain the Hospitalia plans, a phone line and video calls with English-speaking advisers. The general conditions are still in French. You must be a Partenamut member and fill in a medical questionnaire, and a pre-existing condition will not get single-room supplements refunded.

Partenamut suits expats in Brussels or Wallonia who want someone to explain the plan in English and a mid-priced single-room cover. If you belong to another mutuality, you would have to switch fund first.

2. Helan (Hospitalia) 5.9/10

Ranked 2 of 7 rated providers in this guide.

Best for: Expats in Flanders or Brussels who want to read their hospital cover in English.

Helan sells exactly the same Hospitalia range as Partenamut, also insured by MLOZ Insurance: Hospitalia Plus, Care, Smart. What changes is the fund you join and the way it is explained. Helan’s English product pages are the most complete we found for hospital insurance.

Price: check current price (2026 price not on English pages). Helan’s English cost table is dated, so we did not record a current figure. Since the insurer and the formulas are the same as at Partenamut, ask Helan for its current tariff and compare it with Partenamut’s before deciding.

Single room: Yes, room up to €125/day; €150 deductible in some hospitals. Hospitalia Plus has no annual ceiling, while Smart is capped per year. Before and after a stay: 2/4 months (Smart), 2/6 months (Care, Plus). Waiting period: 3 months (pregnancy up to 9); Helan waives it after an accident or if you had similar cover before. Maximum entry age: 67 (Care, Plus); no limit for Smart.

Helan includes Hospitalia Assist, which pays for household help, childcare or medical transport after a stay, and implants and medicines on hospital invoices are covered within stated ceilings. You can buy online, with a medical questionnaire and, for some conditions, a medical report.

Helan suits expats in Flanders or Brussels who want to read their cover in English and are already Helan members, or plan to join. We did not confirm that phone support or contracts are in English.

3. Solidaris (Wallonia) 4.8/10

Ranked 3 of 7 rated providers in this guide.

Best for: Budget-minded Solidaris members happy with a double room.

Solidaris Assurances is the mutual insurance company of the socialist mutualities, and its plans are reserved to Solidaris members. The Wallonia tariff is published. Price: €6.93/month (€83.16/yr), Hospimut double-room plan, joined at 30-39 (2026).

That is the cheapest published premium in this guide, but it is not comparable with the others. The base Hospimut plan covers a double room. Single room: Option only (room supplement up to €100/day). Once you add the Optio option needed for a single room, the price climbs well above the base figure; the price cell for Solidaris shows the top option too. Before and after a stay: 60/90 days with Le Complément Hospi add-on.

Children can be covered free when their parents also hold Hospimut, under conditions. Hospitalisation abroad, implants and ambulance are covered within stated ceilings. Waiting period: not published. Entry age: not published.

Everything is in French: No (French). The tariff we read is for Wallonia; Brussels members belong to Solidaris Brabant. Solidaris suits budget-minded members who are happy with a shared room, and families with children.

4. Ethias 4.3/10

Ranked 4 of 7 rated providers in this guide.

Best for: People who want a private, lifelong single-room plan with a published price list.

Ethias is a Belgian insurer, so its plans do not depend on which mutuality you joined. It publishes a full price list by age and region, which is rare. Formula: Hospi Quality+ (Hospi Quality: €292.42/yr). Price: €572.54/year - Hospi Quality+, age 30-34, Brussels-Capital (2026, taxes incl.).

That figure is the Brussels column. The same plan costs less in Flanders and more in Wallonia, so read the tariff for your own postcode. It is a single-room plan, the most expensive published here, but it is also a lifelong contract under Belgian law with worldwide cover in any hospital setting.

Single room: HQ+: yes (€444.80 deductible in non-certified hospitals); HQ: Joker for serious illness. Waiting period: 3 months. Maximum age to join: 64 (Hospi Quality), 69 (Hospi Quality+). Costs before and after a stay: not published on the pages we read. Both formulas refund outpatient costs linked to a list of serious illnesses, with or without a hospital stay.

To cancel, Ethias asks for a registered letter well before 1 January. Pages and tariffs are in French and Dutch only: No (French/Dutch). You can subscribe online.

Ethias suits people who want a plan independent of their mutuality, with a price they can check in advance, especially in Flanders where the regional tariff is lowest.

5. DKV 4.2/10

Ranked 5 of 7 rated providers in this guide.

Best for: People who want an individual private hospital plan independent of their mutuality.

DKV is a specialist health insurer, authorised only for sickness and assistance insurance. Its three plans are Hospi Essential, Hospi Comfort, Hospi Premium +. Price: check current price (online simulator asks postcode and birth year). The simulator gives an indicative premium; your health is assessed afterwards.

Single room: Single room: €217.75 own share per stay (Essential, Comfort); full refund (Premium +). Hospi Essential does not cover day hospitalisations in a single room. Before and after a stay: 30/90 days (Essential, Comfort), 60/180 days (Premium +). Waiting period: 12 months for dental treatment. Entry age: not published.

The top plan has no annual ceiling on serious-illness costs, and the two higher plans cover urgent hospital stays outside the EU. With the Medi-Card, DKV pays the hospital directly. A newborn must be declared within a short deadline after birth to join the contract.

Two points for newcomers. First, DKV halves its refund when Belgian statutory insurance pays nothing on the bill, so join a mutuality before you rely on DKV. Second, the website does not work in English: No (dkv.be/en redirects to the Dutch site).

DKV suits people who want strong individual cover that stays with them whatever their mutuality or employer, and who can read Dutch or French.

6. Liberal Mutuality (LM) - Hospitaal-Plus 4.1/10

Ranked 6 of 7 rated providers in this guide.

Best for: LM members wanting a moderately priced capped plan.

The Liberal Mutuality sells Hospitaal-Plus plans through its own mutual insurance company, and only to its members. Formulas: LMcomfort+ / LMcare+. Price: €24.07/month (€288.84/yr) - LMcomfort+, age 30-49, joined at 0-49 (tariff from 01/01/2026).

That price looks lower than Hospitalia Plus at the same age, but the cover is not the same: both Liberal Mutuality plans have an annual ceiling, and the single-room terms were not on the pages we read: not published. Before and after a stay: Up to 3 months after. Waiting period: not published.

Children up to a certain age are covered free if a parent holds the same product. English: No (Dutch/French).

These plans suit Liberal Mutuality members who want a moderately priced, capped plan. For a newcomer choosing a fund partly for its hospital cover, the missing single-room details make it hard to compare.

7. AG Insurance 3.9/10

Ranked 7 of 7 rated providers in this guide.

Best for: Brussels residents who want a single room in the more expensive hospitals (SL option).

AG Insurance sells AG Care Hospitalisation through brokers. Price: check current price (via broker or AG simulator). You need a broker to buy it, and AG publishes no price list.

Who can join: 69 (must be domiciled in Belgium and affiliated to a Belgian mutuality). Waiting period: None after medical acceptance, which is unusual and useful if you have just arrived. Before and after a stay: 1 month / 3 months (Delta: 2 / 6).

Single room: Full refund in most hospitals; 50% own share in some Brussels hospitals without SL option. That detail matters in Brussels. Some of the more expensive hospitals, mostly in Brussels, are only fully covered with the SL option; without it you pay half the single-room costs yourself there. Costs linked to a list of serious illnesses are refunded even without a stay, and cover and assistance abroad are included.

English: No (French/Dutch). The broker channel and the medical acceptance make it the least simple option for a newcomer.

AG suits Brussels residents who want a single room in any hospital, including the most expensive ones, and are happy to go through a Belgian broker.

Christian Mutuality: not checked

The Christian Mutuality (MC in French, CM in Dutch) sells hospital plans to its members through MC Assure and Verzekeringen CM-Vlaanderen. We could not read them: mc.be and cm.be both refused our access when we checked on 2 November 2026, so we have no price, room terms or waiting period. It is listed as not rated under the table rather than scored: this reflects missing data, not a judgement of the plans. If you are a member, read the current offer on the Christian Mutuality site.

Situations that change the choice

If you have just arrived

Join a mutuality before anything else. Then check whether your employer has a group plan. If you need individual cover quickly, remember the waiting periods: AG Insurance has none after medical acceptance, while most mutuality plans and Ethias apply one. Keep proof of any previous hospital insurance, even from abroad, and ask whether it shortens the waiting period.

If you live in Brussels

Brussels has several expensive hospitals where single-room supplements are high. Look at the hospital you would actually use. AG’s SL option was designed for them; Hospitalia Plus and Ethias apply a deductible in some hospitals. Ethias’s Brussels tariff sits between Flanders and Wallonia.

If you want to read your contract in English

Partenamut and Helan are the only sellers we found with English pages on hospital insurance, and both sell Hospitalia from MLOZ Insurance. The general conditions remain in French or Dutch.

If you plan to stay only a few years

Mutuality plans keep your entry bracket, which matters less if you leave in two years. A group plan from your employer, or a plan you can cancel cleanly, may be enough. Ethias requires notice by registered letter before 1 January.

If you have a family

Solidaris and the Liberal Mutuality cover children free under conditions when a parent holds the plan. DKV requires newborns to be declared quickly. Ask about pregnancy: Helan’s waiting period is longer for it.

If you have a health condition

Declare it. DKV lists pre-existing conditions among its main exclusions; Partenamut covers them but without single-room supplements. If your employer has a group plan, ask HR how it treats existing conditions before you look for individual cover.

Step by step: taking out a hospital plan

  1. Join a mutuality and wait until you are registered.
  2. Ask your employer whether you already have group hospital cover, and for whom.
  3. Decide on room type. Single room or shared room changes the price more than the insurer does.
  4. Compare the same profile. Same room type, your age, your postcode. Use the tariffs Ethias, Partenamut and Solidaris publish, and request quotes from DKV and AG.
  5. Fill in the medical questionnaire honestly, and keep a copy.
  6. Note the waiting period and the date cover starts.
  7. Keep the insurer’s card or app with you. Some plans pay the hospital directly; for others, you send the bill.

If you disagree with a refusal, complain to the insurer first, then to the Ombudsman des Assurances, which examines insurance disputes.

Our verdict

Partenamut (Hospitalia) comes out ahead in the table. For English-speaking newcomers, Partenamut is the one to start with: it sells the Hospitalia single-room plans from MLOZ Insurance with a published tariff and an expat desk that explains them in English. Helan sells the same range with complete English pages and suits members in Flanders or Brussels, once you have checked its current price. Solidaris has the lowest published premium but for a shared room; it is a good budget choice if that is what you want. Ethias is the most transparent private insurer and the best pick if you want cover independent of your mutuality, especially in Flanders. DKV offers strong individual plans for Dutch or French readers, the Liberal Mutuality suits its own members, and AG’s SL option is the one to look at if you want a single room in Brussels’s most expensive hospitals. Before any of them, ask your employer: the best hospital plan may be the one you already have.

Frequently asked questions

Do I need a Belgian mutuality before buying hospital insurance?

For mutuality plans, yes: you must be a member of the same fund (e.g. Helan, LM). AG Insurance also requires Belgian domicile and affiliation to a Belgian mutuality, and DKV cuts refunds to 50% when the statutory insurance pays nothing. Source.

Is there a waiting period?

Usually yes: 3 months at Partenamut/Helan (Hospitalia) and Ethias; AG Insurance has none once you are medically accepted. Source.

Why does the price depend on my age when I join?

Mutuality plans such as Hospitalia set the premium bracket by age at affiliation, so joining younger keeps a lower bracket; Ethias prices by current age category and region. Source.

Are pre-existing conditions covered?

DKV lists them as a main exclusion; Partenamut keeps the premium but does not refund single-room supplements for a pre-existing condition. Source.

Which plans cover a single room in expensive Brussels hospitals?

AG offers an SL option for the more expensive hospitals mostly in Brussels; Hospitalia Plus and Ethias HQ+ apply a deductible in some hospitals (€150 and €444.80 respectively). Source.

Where can I complain about an insurer?

The Ombudsman des Assurances examines insurance disputes and tries to reconcile the parties. Source.
